Game wise, the uncut version of Manhunt 2. Holy 'friggin bejesus.

However, there's also Brutal Doom and Grezzo 2... however, these are both DOOM II mods.

Some of the bosses in Grezzo 2 include Hitler with his pants down, the flying head of Osama bin Laden, and a mecha T-Rex that shoots exploding fireballs. 'Nuff said.
